Dr. Bhavik Patel PharmD is the supervising pharmacist at Hoyt Pharmacy. Dr. Patel pursued a career in healthcare. because of his desire to impact patients’ overall health. He recognized that dealing with multiple medications and new health conditions can be overwhelming and challenging for many patients. He strongly believes that a team-based approach can help achieve health goals. Primarily working in an ambulatory care setting, Dr. Patel assists with immunizations for the community, and aids in the management of hypertension, diabetes, cholesterol, asthma and many health conditions. He also works closely with providers in the community to improve health outcomes. He is an advocate for patient education, helping them understand their medications. and treatment better.
Dr. Patel shares this about his approach, “I believe that including the patient in their health care is integral for decision making. Educating my patients helps them become their own health advocate for better self-management and attainment of their health goals.” For him, it is crucial to listen to the patient’s concerns during encounters and conduct a thorough medication reconciliation. This way, he ensures that the patient’s treatment regimen is optimal and any potential problematic drug interactions are addressed.
